As a courtesy to you, upon verbal verification, we will accept assignment for your insurance benefits. Please note, however, verification is not a guarantee of paid benefits.

We welcome the chance to let your insurance help pay the bill, but our services are rendered to the patient, not the insurance company. Insurance assignment is accepted for a period not to exceed forty-five days. If the insurance company has not paid within forty-five days of filing the insurance, the patient is responsible to pay the balance in full.

Please remember the patient is fully responsible for all fees charged by this office regardless of your insurance coverage.

We will send you a monthly statement. Most insurance companies will respond within four to six weeks. Please call our office if your statement does not reflect your insurance payment within that time frame. Any remaining balance after your insurance has paid is your responsibility. Your prompt remittance is appreciated. We can make arrangements for a monthly payment plan, but that must be implemented prior to the actual procedure.

For your convenience, we accept Visa, MasterCard, Discover and American Express. We offer numerous financing options, Care Credit and Capital One Fee Plan. If you have questions regarding your account, please contact your account manager listed on your billing statement. Many times, a simple telephone call will clear up any misunderstandings.

Please note in the unfortunate event of a returned check, Northwest Oral & Maxillofacial Surgery will access your account with a $25.00 insufficient fee.
