In order to process your information more efficiently for your first appointment we ask that you assist us in your insurance coverage. Insurance coverage varies between dental/medical and we will be calling both to obtain the maximum coverage for your procedure.
Our office requires a $50 to $150 deposit to schedule surgery. The deposit will be applied to patient services costs related to the scheduled surgery. Surgeries cancelled less than 48 business hours to the date of surgery will forfeit the deposit.
While we welcome the chance to let your insurance help cover the cost of services, however services are rendered to the patient, not the insurance company. We ask that you keep in contact with your insurance carrier so the claim will be processed as quickly and efficiently as possible. As a courtesy to you, upon verbal verification, we will accept assignment of your insurance benefits. Verification is not a guarantee of benefits.
Insurance assignment is generally accepted for a period not to exceed forty-five days. If insurance has not paid within forty-five days of filing the insurance, the patient is expected to pay the balance in full.
If insurance pays less than the estimate, you will be billed for the remaining balance owed. If insurances pays more than the estimate, we will may you a refund check.
We accept cash, check, all major credit cards (Visa, Master Card, Discover, American Express) and work with several financial companies (examples—Care Credit).
